Tank size, orientation and location?
    (i.e. I have a 15' X 7' aboveground, horizontal tank, located inside.)

Type of product contained and amount still in tank?
    (i.e. Contained product is Benzene, Latex, Gasoline, etc.... and there is 6 inches of product still in the tank.)

Reason for work? (i.e. Cleaning, Testing, Waste Removal)

Tank entry point? (i.e. There is a 24" manhole on the top of the tank.)

Do you have a MOE waste generator number and what waste class?
    (i.e. Yes, we are licensed for 251L)
